Please bring the following to your appointment:

A list of ALL your current medications and dosages
Driver’s License or state ID card
Insurance card (s)
All of your prescription/over-the-counter glasses and contact lenses
Your co-pay and/or deductible (if not met)
Contact lens evaluation fee
Plus $40 for the Refraction* fee
Telephone number for your Pharmacy
A written list of questions you have for the doctor

You do NOT have to bring your medical records from your previous eye doctor. If we determine they are needed, we will ask you to sign a medical records release at our office.

For routine vision exams, 0ur experienced ophthalmic technicians will conduct several pre-tests before you see the doctor. We will dilate your eyes, which will blur your close-up vision and possibly your distance vision. Most patients will still be able to drive. However, if you have a concern, please bring a driver with you to your appointment.

We will perform a vision test and refraction. We will also ask you questions regarding your general health, as this is related to your eye health. The refraction determines your current corrected vision and also can provide information about the prescription you may need.

* You may call your insurance carrier to determine if they pay for refractions. All fees are subject to change without notice. (Medicare does NOT cover refractions.)
